To receive a free digital scale ($45 value) by mail, send ALL of the following:

	A printout of the completed Rebate Submission Form (below)
	The original UPC bar code from the product box lid
	(UPC barcode must show the model number BB-SSC10-WZ)
	A legible copy of your receipt. Make certain that it shows
	the store’s name, location, date of purchase and purchase price.

Zojirushi and the store where purchased are not responsible for lost, late, illegitimate, incomplete, illegible, damaged, misdirected or postage-due requests or if the rebate is returned to Zojirushi for any reason. Rebate claims may not be assigned or transferred. Only the original purchaser is entitled to receive a rebate. Claims from dealers, distributors, or their employees or from warehouse facilities, groups, clubs or organizations will not be accepted. Zojirushi reserves the right to audit any claims, request additional information regarding this claim and to nullify any claims which cannot be substantiated. Omission of sales receipt or any other required forms and information will result in a denial of your rebate. Purchases through online auction sites will not qualify. Purchases of refurbished models do not qualify for this rebate. Purchases that already included a free digital scale will not quality. Not valid with any other promotions.
